Do you have a personal history of breast cancer?

Have you ever been diagnosed with atypical lobular hyperplasia, atypical ductal hyperplasia or lobular carcinoma in situ?

Do you have one or more relatives who have been diagnosed with breast cancer under the age of 50?

Do you have a family history of breast cancer AND ovarian, pancreatic or prostate cancer?
